We will thoroughly evaluate patients’ concerns and treat the patients’ illnesses. We want to assist patients in discovering their goals of care and to establish their advance care plans. We want to develop a relationship with our patients that is based on open communication and trust, so that we can work together to accomplish a medical care plan that is medically sound and also in-line with the patients’ goals of care. We want to help maintain, improve and/or restore patients’ level of function and quality of life.
